Medication Refill Request Form

To request a refill by phone, call our office at 818-501-5155 to speak with one of our team members, or  press “2” to leave a message.

We offer the following options to receive your medication:

  1. Pick up from our office during regular business hours Mon-Thurs 8am-5pm: Please call our office in advance to have medications ready for pick-up from our reception team.
  2. Send the prescription to your local pharmacy (Costco, Walgreens, CVS, etc.).  Please call our office with a phone number and address to your local pharmacy.  Some eye medications (Tacrolimus, EDTA, Cidofovir) are not available at a regular pharmacy (Costco, Walgreens, etc.), as they are made at a compounding pharmacy. If you have a compounding pharmacy in your area, we can send the medication prescription there as well. We cannot provide pricing information on medications at other pharmacies.
  3. Online pet pharmacy (Allivet, PetMed, Chewy, etc.).  Log-in to your online pharmacy account, select the medication that we have prescribed for your pet and add it to your cart. Designate our clinic to receive the medication authorization request. Once we have received the medication request and confirmed its accuracy, the pharmacy will be authorized to send you the medication.

We highly recommend calling around for prices before choosing a pharmacy, as prices can vary widely.

Please note: By federal law, we must see patients once a year to renew prescriptions. We are unable to refill prescriptions if it has been over 1 year since your pet’s last exam. If you are unable to return to our clinic for an annual exam, please contact your pet’s primary care veterinarian for refills.
